Think of two incidences in the last three years/at college/at school where you had a major success or a major failure. Describe these. Now explain the cause. Was it something about you yourself, or the situation you were in? Is the cause pretty consistent and stable or highly variable? Third, does the cause have a major impact on all aspects of your life or is it really very confined to very specific situations?
