Abstract

The rapid emergence across the globe of large-scale antiretroviral therapy (ART) programs for people living with AIDS has been nothing short of a revolution. In the space of just four years, the number of people in low- and middle-income countries receiving this life-saving treatment increased fivefold, from 240,000 in 2001 to 1.3 million at the end of 2005 (WHO, 2006a). By December 2008, the number had expanded yet further, to over 4 million, of whom around 3 million were in sub-Saharan Africa (WHO, 2009a).
