Abstract

Deciding how many consultants are required in a specialty is obviously essential for the provision of the service but is also vital in order to plan the number of training posts needed to fill future consultant vacancies. If too few are trained, consultant numbers cannot expand or posts may be filled with untrained individuals but if there are too many trainees, there is a risk of unemployment. Because the length of training is four to five years and financial planning is done a year in advance, it is necessary to try to predict requirements six or seven years into the future. This has been a major cause of difficulty over the years (in all specialties.) Although at times there have been fears that there may be too many training posts, the problem in A&E has always been too few trained individuals to fill consultant posts. In part this may be because recommended consultant numbers seem to increase when there are plans to reduce the number of trainees: at times the tail (trainee numbers) appears to wag the dog of consultant numbers.
