Abstract

Chondroid matrix is seen in lesions comprised of chondrocyte proliferation. The radiographic appearance can be described as ring-and-arc, punctate, curved, stippled, or coarser flocculent foci of calcification. The radiographic presence of chondroid matrix implies mature cartilage production; however, this may be seen in both benign and malignant lesions. A benign enchondroma may be radiographically identical to a low-grade chondrosarcoma in the absence of aggressive features such as cortical destruction and soft tissue extension. The location of the chondroid matrix within the bone and the age of the patient help narrow the differential diagnosis. A chondroid lesion in the epiphysis of a pediatric patient is almost certainly a chondroblastoma, whereas the same lesion in the third or fourth decades of life may represent a clear cell chondrosarcoma. An intramedullary lesion in the metaphysis of an adult patient may represent an enchondroma or chondrosarcoma.
