Abstract
Within modern community-based healthcare in Australia, care assessors are employed by community-based healthcare organisations to facilitate the assessment and integration of healthcare services into the context of the home of people needing such care. That integration process brings with it challenges and complexities that we need to be aware of and that need to be managed from the outset if community-based healthcare is to be successful for all concerned.
