Prevention of Cancer in Barrett’s by Ablation

Abstract

The problem of cancer in patients with Barrett’s is well described in the foregoing chapters of this book. It is clear that the outcomes of treatment for established carcinoma of the esophagus are very poor. Reliance on surveillance programs to achieve adequate early detection of cancer followed by submission of a patient to the major surgery required for esophageal resection is neither an attractive option, nor one of proven value. A potentially attractive alternative is to attempt to prevent the development of carcinoma by the destruction of the Barrett’s epithelium in a reflux controlled environment, and to stimulate the regrowth of a normal squamous lining. Prevention of cancer offers the greatest opportunity for improvements in both survival and quality of life of patients with Barrett’s esophagus. This chapter will describe techniques that have either been used or are being developed for the ablation of Barrett’s epithelium with the intention of cancer prevention. The importance of reflux control, characteristics of the neo-squamous epithelium, current issues/controversies in Barrett’s ablation and future strategies in Barrett’s ablation will also be explored.
