Abstract
In our area of the world replacement of the mitral valve has become a widely accepted therapeutic procedure since 1960 when Starr and Harken independently successfully first replaced mitral valves and established the concept of functional rather than anatomical repair of mitral valve disease. It is evident from other experience presented in this volume that there remains a proper role for reconstruction of the mitral apparatus, but at our institution we encounter perhaps a somewhat different population of patients. In less than 5% of cases at our institution does autologous tissue lend itself to haemodynamically acceptable plastic revision. This may be due in part to the age distribution of patients whom we have treated, since the average age in our series has been 56 years and has ranged up to 89 years. This is in contrast to most other series in which average patient age has usually ranged between 40 and 50 years.
