Abstract
After periods ranging from 4 months to 2 years on 9–10 1/2 hours haemodialysis weekly, six patients became vaguely unwell over a 2-week period. They developed increasing dyspnoea, anterior chest pain, then pericardial friction. Cardiac tamponade occurred in three cases. The prodromal illness had features not typical of either bronchopneumonia or pulmonary oedema and all had evidence of left ventricular hypertrophy and ischaemia on ECG prior to the illness although only two had previous hypertension. The mean pre-dialysis blood urea at the onset of pericarditis was 39 mmol/1, this being 26% higher than previous levels. These cases represent an incidence of pericarditis of 8% in our dialysis population. Various authors1 quote an incidence of 10–17% in patients maintained on ‘adequate’ dialysis.
