‘ANCA’ in liver diseases

Abstract

Antineutrophil cytoplasmic antibodies (ANCA) are heterogeneous autoantibodies directed against constituents of neutrophilic granulocytes. Since their first detection over 15 years ago, they have become valuable diagnostic tools for a variety of disorders. There is no doubt that ANCA play an important role in the diagnostic and therapeutic management of patients with systemic vasculitides such as Wegener’s granulomatosis or microscopic polyangiitis. Recently,’ ANCA’ have been described at particularly high frequencies in sera of patients with chronic inflammatory bowel diseases such as ulcerative colitis and Crohn’s disease or autoimmune liver disorders such as primary sclerosing cholangitis and autoimmune hepatitis. However, the role of the so-called’ ANCA in these disorders has still to gain consensus. With particular respect to this controversial discussion, this chapter aims to review the clinical and diagnostic relevance of such autoantibodies in autoimmune liver disorders.
