Abstract
The issues that pertain to arthritis in pregnancy differ from those of the other chapters in this book by virtue of the finite duration of the gestational period. The key issues relate to the effects of pregnancy on the musculoskeletal condition and the effects of the musculoskeletal condition on pregnancy. This broader categorization is preferred to that which limits the discussion to the interaction between pregnancy and the articular condition, since in some disorders, e.g., SLE, the severity and progression of major organ involvement outweighs any articular effects. In addition to discussing gestational effects in the major musculoskeletal conditions, this chapter also examines, where relevant, issues relating to fertility, the post-partum period and drug effects in pregnancy and lactation. Although an attempt will be made to discuss the outcomes according to the paradigm advanced by Fries1, it is of note that economic outcomes (dollar costs) are poorly delineated, most investigators exploring the incidence of maternal and fetal fatality (death), the fluctuating severity of pain (discomfort) and suffering (disability), and the effects of pharmacologic therapy (iatrogenic effects).
