Abstract

This communication discusses the role of thrombosis in relation to plaque growth and in the pathogenesis of acute ischaemic syndromes.

The presence of thrombosis on an arterial surface leads to a swift and striking proliferation of intimai smooth musde cells and an increase in the extracellular matrix proteins which these cells produce. Smooth muscle proliferation is almost certainly mediated by binding of component growth factors such as PDGF to the muscle cells, and the events that follow such binding and the effects on the PDGF system of hyperdholesterolaemia are discussed.

Thrombi occurring in relation to the pre-existing plaques vary from those which are only recognisable ultramicroscopically to those which are visible either on angiography or naked-eye examination of post-mortem samples.

Angiographicaly identifiable thrombi are found in a very high proportion of cases of acute myocardial ischaemia, and the morphological characteristics of any coronary artery plaques which are likely to lead to such thrombosis are briefly described.
