Abnormal Coronary Vasoconstrictor Responses to Serotonin in Patients with Ischemic Heart Disease

Abstract

The coronary vasomotor responses to acetylcholine, serotonin (5-HT) and to isosorbide dinitrate were assessed by quantitative coronary arteriography in 31 patients.

Acetylcholine induced constrictions in diseased coronary arteries (n=15). In the patients with smooth normal coronary arteriograms, acetylcholine caused dilatations in 7 patients (smooth dilators) and constrictions in 9 patients (smooth constrictors). 5-HT evoked no significant diameter changes in the smooth dilators (+1.4 ± 4.1%), but in the smooth constrictors and in the patients with diseased coronary arteries 5-HT caused potent constrictions. The vasoconstrictor responses to 5-HT were similar in patients with minimal (-26.5 ± 4.7%) and with advanced atherosclerosis (-30.9 ± 5.3%). Isosorbide dinitrate reversed rapidly the vasoconstriction induced by 5-HT.

Both in early and advanced atherosclerosis, impairment of endothelium-dependent vasodilation causes a hypersensitivity of the coronary artery to the vasoconstrictor stimulus of 5-HT.
