Abstract
Atrial fibrillation without structural heart disease[1] or pre-excitation syndrome[2] is an arrhythmia which does not markedly affect life expectancy but may strongly alter exercise capacity and quality of life. This arrhythmia is highly prevalent among cardiac arrhythmias, particularly in older people, and becomes frequently manifest without demonstrable cause (’lone atrial fibrillation’). In circumstances of drug refractoriness of disabling atrial fibrillation today two alternative electrophysiologic interventions are available: His-bundle ablation followed by chronic cardiac pacing[3,4], or the surgical exclusion of the anatomical substrate of atrial fibrillation[5,6,7].6
