Abstract

Digitoxin is a cardiac glycoside with the same actions and uses as digoxin. It is slower and longer acting than digoxin with variable excretion leading to a plasma half-life of from 3 to 16 days. Metabolism of 50-80% of the dose is by hydrolysis, hydroxylation and conjugation. Several metabolites which include digoxin are active. Plasma level monitoring is used as toxic symptoms can be difficult to distinguish from those of under-dosage. The therapeutic range in plasma is 10-30 ng/ml.
