Abstract

The true rate of spontaneous abortion in early pregnancy is not well known. A prospective study (Miller et al., 1980) in 197 women based on the presence of elevated urinary human chorionic gonadotropin (hCG) during the luteal phase of the menstrual cycle shows evidence of conception in 24% (152) of 623 menstrual cycles. Of these, only 57% (87) advanced beyond 20 weeks’ gestation. Spontaneous abortion was apparent in 14% of 102 recognized pregnancies. Of the lost conceptions only one-fourth are clinically recognized abortions. The conceptions identified by raised β-hCG levels are considered to be postimplantation conceptions. The causes of the early preamenorrhoic abortions are yet to be interpreted. Clinically registered abortions are reported at lower levels, between 10 and 15%.
