Abstract

Repair of a completely missing sound conduction system has been and remains the greatest challenge in tympanoplastic surgery. The excellent hearing enjoyed by those whose chronic ear disease has healed with a natural myringo-stapediopexy serves as a goal for otologic surgeons. That we have not yet attained this goal is attested to by the many solutions offered in our past and present literature. This author’s experiences over the past twenty years is offered to illustrate the problem. The solution will not be found herein, only a map of false starts.
