Abstract
The study of the dementias must be viewed in the context of the physiology of normal ageing. One of the questions that dominated dementia research for a time was whether dementia of the degenerative type constituted an acceleration of the normal ageing process. Other questions in the pathophysiological study of the dementias relate to the relationship between cerebral ischemia and the pathogenesis of multi-infarct disease and the possibility of in vivo aetiological diagnosis using an objective biological test. PET has addressed these questions in a number of ways. Studies of energy metabolism have appeared since 1981, firstly in terms of oxygen metabolism and haemodynamics and then glucose metabolism. Correlations of energy metabolism with type and severity of dementia have been attempted. The distribution of abnormalities of energy metabolism has been correlated with the functional disturbances measured clinically. Specific patterns of disturbed metabolism have been sought in relation to cortical and subcortical dementias and dementias of vascular aetiology. Finally, studies of neurotransmitter function have been undertaken with the dual aim of elucidating pathogenic factors and also developing more specific diagnostic markers.
