Arterial Infusion in the Treatment of Osteosarcoma

Abstract

Intra-arterial cis-platinum was introduced into the treatment of human cancer in 1977 in a patient with intransit metastases of malignant melanoma.(1) After an impressive response in that patient, a broad Phase II study was initiated. (2) One of the earliest patients on that study had an extensive malignant fibrous histiocytoma of the pubis with a soft-tissue mass which filled more than half the pelvis. The tumor had progressed in size despite therapy with radiation, 70 Gray, plus weekly systemic adriamycin. When she presented to us, she had extensive peripheral edema and was considered inoperable for a hemipelvectomy. We reasoned that since cis-platinum had some reported activity against osteosarcoma, malignant fibrous histiocytoma of bone might respond in a similar way, and that since the tumor was localized to the pelvis, the intra-arterial route would be optimal. The patient was treated, returned home to California, and called two weeks later to report the development of a new mass.
