Abstract

In most countries, human bladder cancer is usually transitional cell carcinoma which can be classified into at least two clinically distinct types: papillary, superficial carcinoma and non-papillary, invasive carcinoma. The former frequently recurs after treatment, often appears as a multifocal lesion in the bladder, but is associated with an excellent prognosis. The non-papillary tumor is invariably invasive, frequently associated with metastases, and therefore, usually has a poor prognosis (16, 56). Approximately 80% of transitional cell bladder carcinomas start as papillary tumors and only 20% begin as sessile growth. A high proportion of patients who develop an invasive bladder tumor already have such a lesion at the time of first presentation. Also, both types of tumor can be present simultaneously in the same bladder or sequentially. The reason why two different types of carcinoma can develop in the urinary bladder is not known.
