Abstract

Acute rejection is not a steady phenomenon, but occurs in sporadic waves, extending over a few days or a week or two. Since the introduction of cyclosporine (CsA) as an immunosuppressive agent, severe acute rejection episodes have become relatively rare (seen in less than 2% of endomyocardial biopsies at our center)1. Mild acute rejection (cellular infiltration) is seen fairly frequently, but many groups would not increase therapy in such cases. It is impossible to predict whether or not any individual patient will experience episodes of rejection and, when it occurs, it may be impossible to make the diagnosis on clinical evidence until it is extremely advanced.
