Long-term outcome of peritoneal dialysis and hemodialysis in pediatric patients

Abstract

Pediatric patients have been taken onto dialysis treatment for the last 30 years and comprise approximately 2% of any national dialysis program1. Despite this, long-term outcome data for this special group are scarce, reflecting the relatively small numbers involved and the difficulties in tracking these patients beyond adolescence. Pediatric nephrologists are now starting dialysis treatment in infants and children with the expectation that a successful renal transplant is an achievable goal and will provide the best survival and quality of life for their patient. However some children will require long-term dialysis or may return intermittently to dialysis during the course of their end-stage renal failure (ESRF) management. This chapter reviews the available pediatric outcome data including some long-term observations from our unit and attempts to use the larger adult dialysis outcome experience to inform our pediatric practice. The multiple factors that may influence outcome for children on dialysis and in particular, those which can potentially be modified are discussed.
