Samenvatting
In de eerstelijnsgezondheidszorg is de huisarts het eerste aanspreekpunt voor de patiënt met chronische pijn. De huisarts stelt in zo’n situatie allereerst vast of er een somatische oorzaak is die de klachten kan verklaren. Indien er sprake is van chronische pijn, dan brengt de huisarts op basis van een multifactoriële analyse het pijnprobleem in kaart. Een biopsychosociaal perspectief is daarbij van belang. Vervolgens kiest de huisarts de meest passende behandelroute voor iedere individuele patiënt. Deze varieert in de eerstelijn van educatie door de huisarts alleen, ondersteuning door de praktijkondersteuner GGZ (POH-GGZ), tot eerstelijnsbehandeling bij een fysiotherapeut of oefentherapeut of een gezondheidszorgpsycholoog (GZ-psycholoog) in de generalistische basis-GGZ. Wanneer de huisarts op basis van zijn inventarisatie vermoedt dat het gaat om matig complexe problematiek, zou hij volgens de zorgstandaard idealiter moeten kunnen verwijzen naar een multidisciplinair team in de eerstelijn. De leden van het team delen dezelfde biopsychosociale visie en zijn allen deskundig op het gebied van chronische pijn. Het team verricht brede multidisciplinaire diagnostiek, geeft pijneducatie en behandelt bij voorkeur in de eerstelijn, in nauwe samenwerking met de tweedelijn. Bij hoog-complexe problematiek verwijst dit team gericht naar de tweede of derdelijn.
