Samenvatting
Hepatitis B behoort niet tot het verleden. Het hepatitisvirus bestaat uit een partieel dubbelstrengs circulair DNA met een kern (core antigen of HbcAg) en een e-antigeen (HbeAg). Dit laatste bepaalt de virulentie. Acute hepatitis verloopt symptoomloos of als griep. De patiënten genezen in 90 tot 95 % volledig. Complicaties zijn leverfunctieverlies, levensbedreigende bloedingen uit varices en het primair levercarcinoom. Voor hepatitis B zijn verschillende vaccinaties op de markt: te geven op dag nul, na één maand en na zes maanden. Na drie injecties zijn kinderen in 95 % beschermd en gezonde volwassenen onder de 40 jaar in 90 %. Een verminderde respons tonen personen boven de 40 jaar, rokers, personen met een BMI hoger dan 30, bij een gestoorde cellulaire immuniteit (hiv), dialysepatiënten, volwassenen met het downsyndroom en hepatitis C-patiënten, soms is de reden onbekend. De non-responders reageren in 89 % goed op het nieuwe vaccin Fendrix®. Bij immuungecompromitteerden is dit gemiddeld 38 %.
