Anale pijn- en disfunctionele klachten en de relatie met het prikkelbaredarmsyndroom

Samenvatting

In dit hoofdstuk worden anale pijn en disfunctionele klachten in relatie tot het prikkelbaredarmsyndroom besproken. Klachten van het achterste compartiment kunnen niet los van klachten van de rest van de bekkenbodem worden gezien. Vaak is een ‘snelle’ oplossing niet beschikbaar. Organische aandoeningen moeten uitgesloten en behandeld worden. Bij persisterende klachten moeten functionele oorzaken overwogen worden in de wetenschap dat langdurige pijn een catastroferend effect kan hebben en de pijngevoeligheid doet vergroten. Daarnaast is het in kaart brengen van de invloed van de klachten op het dagelijks functioneren van groot belang. Andere aspecten zoals seksuologische klachten worden belicht. Behandeling is bij voorkeur multidisciplinair waarbij naast defecatieregulatie en medicamenteuze pijnvermindering de bekkenfysiotherapeut en de psycholoog een grote rol kunnen spelen.
