Samenvatting
Jicht is een gewrichtsaandoening waarbij een ontsteking ontstaat doordat natriumuraatkristallen neerslaan. De aandoening begint in de regel zeer plotseling, met hevige, aanvankelijk nachtelijke pijn in een (veelal teen)gewricht. Zonder behandeling zullen er herhaaldelijk aanvallen optreden, maar ook bestaat het gevaar voor fikse uraatstapeling onderhuids en in gewrichten. Aangezien bij jicht een te hoog urinezuurgehalte in het bloed voorkomt door een zogenaamde positieve uraatbalans, worden urinezuurverlagende geneesmiddelen voorgeschreven. Urinezuur ontstaat door de afbraak van purine, een onderdeel van het DNA en dus aanwezig in alle cellen. Risicofactoren voor het ontstaan van jicht zijn overgewicht/pogingen tot snel afvallen, verminderde nierfunctie en het gebruik van diuretica, maar ook alcoholgebruik en een eiwitrijke voeding kunnen een rol spelen. De huidige krachtige medicamenteuze behandelingen hebben ertoe geleid dat een purinearme voeding een minder belangrijke plaats heeft in de behandeling, maar toch nog steeds aandacht behoeft. Voedingsadviezen bij jicht bestaan uit een gezonde voeding met aandacht voor het verkrijgen of behouden van een gezond gewicht, beperkt alcoholgebruik, extra vocht en beperking van eiwit- en purinerijke voedingsmiddelen.
