Samenvatting
In dit hoofdstuk wordt de stereotactische operatietechniek beschreven waarmee elektroden voor diepe hersenstimulatie (‘deep brain stimulation’, DBS) in de hersenen worden geïmplanteerd. Deze techniek maakt gebruik van een frame dat aan het hoofd wordt bevestigd, en waarmee een coördinatenstelsel op de hersenen wordt geprojecteerd. Nadat de plaats van de doelstructuur voor DBS is vastgesteld met behulp van MRI, worden de elektroden met een aan het frame bevestigd richttoestel door een boorgat in de schedel op de juiste positie in de hersenen gebracht. Tevens wordt een aantal recente en toekomstige technische ontwikkelingen belicht. Door verbetering van MRI-technieken die de doelstructuren van DBS visualiseren, worden nauwkeurigere implantaties mogelijk. Die MRI-technieken leiden ook tot een toename van kennis over diverse hersencircuits, waardoor nieuwe doelgebieden en nieuwe indicaties voor DBS kunnen ontstaan. Een groot deel van de DBS-operaties wordt onder plaatselijke verdoving uitgevoerd om de effectiviteit en bijwerkingen van de DBS te beoordelen. In de toekomst kan hier mogelijk van worden afgezien, zodat de ingrepen minder belastend voor de patiënt worden. Toekomstige DBS-elektroden zullen het stimulatieveld kunnen sturen, zodat er effectiever wordt gestimuleerd met minder bijwerkingen. Adaptieve stimulatie in een gesloten systeem zal leiden tot een vorm van DBS die automatisch wordt aangepast aan de actuele behoefte van de patient.
