Samenvatting
Het syndroom van Gilles de la Tourette (‘Gilles de la Tourette Syndrome’, GTS) komt voor bij gemiddeld 1 % van de jongeren tussen de 5 en 18 jaar en wordt gekenmerkt door chronische motorische en vocale tics. De klachten kunnen doorgaans goed behandeld worden met bepaalde vormen van psychotherapie en/of medicatie. Bij ernstige en therapieresistente klachten kan behandeling met diepe hersenstimulatie ('deep brain stimulation', DBS) overwogen worden. Uit gepubliceerde ziektegevallen en een heel beperkt aantal onderzoeken wordt duidelijk dat DBS een veilige en effectieve behandeling kan zijn bij een geselecteerde groep patiënten. Bij sommige bewegingsstoornissen is DBS een reguliere behandelmethode, maar behandeling van therapieresistente GTS met diepe hersenstimulatie moet gezien worden als een ‘last resort’ behandeling, die alleen in gespecialiseerde centra kan plaatsvinden. Verder onderzoek moet helpen om meer inzicht te verkrijgen in selectie van stimulatietargets, stimulatieparameters en de effectiviteit van deze behandeling.
