Het beleid bij diabetes mellitus in de palliatieve (terminale) fase

Samenvatting

Als een patiënt met diabetes mellitus het laatste levenspad op gaat, heeft dit consequenties voor de benadering en behandeling van de diabetes. Er vindt een omslagpunt plaats van altijd strak ingesteld zijn naar een beleid met bredere marges tot uiteindelijk (bijna) geen diabetesbehandeling meer. Hierbij blijven patiënten met diabetes mellitus type 1 insulineafhankelijk; er kan getracht worden hen over te zetten op een eenmaal daags insulineregime. Patiënten met type 2-diabetes kunnen bij het voortgaan van de palliatieve stadia toenemend minder medicatie gaan gebruiken. Veelvoorkomende problemen in de laatste levensfase (zoals moeheid, misselijkheid, veranderd bewustzijn) kunnen een uiting zijn van hypo-/hyperglykemie. Corticosteroïdgebruik kan bestaande diabetes ontregelen of een nieuwe diabetes type 2 induceren.
