Samenvatting
Herstel is, naast preventie, het belangrijkste doel van de verslavingszorg. Dit hoofdstuk bespreekt een aantal vragen rondom het begrip herstel. Er wordt onderscheid gemaakt tussen vier vormen: klinisch, functioneel, maatschappelijk en persoonlijk herstel. Daarbij wordt persoonlijk herstel voorgesteld als een mogelijk belangrijke motor van de andere vormen van herstel. Vijf voor herstel belangrijke kwesties worden besproken en de (veelal nog onvoldoende beantwoorde) vragen die in dat verband kunnen worden gesteld. Het gaat om het begrip en de principes van herstel, de ingrediënten en principes van persoonlijk herstel, alsook om de verhouding tussen ervaringsdeskundigheid, de competentie van professionals en wetenschappelijke kennis. Andere thema’s zijn opleiding, onderzoek en de toekomst van herstel. Het hoofdstuk besluit met enkele adviezen aan de verslavingszorg, respectievelijk de daarin werkzame onderzoekers en ontwikkelaars. Men moet aan de slag om de definities met betrekking tot herstel aan te scherpen en nieuwe concepten te ontwikkelen. Verder moet men goed nadenken over wat de consequenties zijn van een verschuivend paradigma waarin de rol van cliënten en ervaringsdeskundigheid belangrijker wordt. Tot slot moet worden nagegaan of bestaande protocollen, richtlijnen en handleidingen nog stroken met nieuwe opvattingen over herstel. Als dat niet het geval is, is aanpassing niet onverstandig.

Dit is een algemeen probleem in de ggz en zeker niet uniek voor de verslavingszorg. Interessant is een recente longitudinale studie van Wunderink e.a. (2013) in Friesland, waaruit bleek dat een onderhoudsdosering antipsychotica na een eerste psychose weliswaar symptomen onderdrukte, maar de kansen op functioneel herstel ondermijnde.

Er zijn uitzonderingen. Bekend, ook in de verslavingszorg, is de dialectische gedragstherapie van Linehan. Hierbij zijn welbewust elementen van persoonlijk herstel (mindfulness) ingebouwd in het behandelprogramma.
