Samenvatting
Wereldwijd worden jaarlijks naar schatting 280.000 kunstkleppen geïmplanteerd. De meeste normaal functionerende mechanische klepprothesen veroorzaken in meerdere of mindere mate obstructie van de flow. Mechanische klepprothesen lekken ook: we onderscheiden hierbij closure backflow (sluitingsterugstroom) die nodig is voor klepsluiting en leakage backflow (lekterugstroom) na de klepsluiting. De mate van obstructie en lekkage is afhankelijk van type en maat van de mechanische klepprothese. Voor een juiste interpretatie van de onderzoeksgegevens bij de individuele patiënt is het van belang om goed op de hoogte te zijn van de kenmerken van de verschillende klepprothesen en om 1–2 maanden na implantatie van de klepprothese een ‘basisechocardiogram’ te maken. Tweedimensionale transthoracale echocardiografie is de voorkeursmethode voor evaluatie van klepprothesen, maar deze wordt gehinderd door akoustische shaduw. Voor volledig onderzoek van de klepprothesefunctie is het daarom soms noodzakelijk een twee- of driedimensionaal transoesofageaal echo, een transthoracaal stress-echocardiogram of een multislice-computertomografiescan te vervaardigen. In dit hoofdstuk worden de uitvoering en de interpretatie van echocardiografische evaluatie van klepprothesen beschreven.
