Samenvatting
De aortaklep bestaat normaal gesproken uit drie klepslippen. Er zijn drie vormen van aortastenose: valvulair, subvalvulair en supravalvulair. De ernst van de aortaklepstenose wordt bepaald door meting van klepoppervlak, energy-loss index, gradiënt en velocity ratio (bloedstroomsnelheid in de linker ventrikel outflowtract ten opzichte van de aortaklep). Achtergrond, uitvoering en valkuilen van deze metingen worden besproken. Aortaklepinsufficiëntie wordt veroorzaakt door een afwijking aan de klepslippen (primaire aortaklepinsufficiëntie), aan de aortawortel (secundaire aortaklepinsufficiëntie), of door een combinatie van beide. De kans op een succesvolle klepreparatie hangt af van het mechanisme van de aortaklepinsufficiëntie: aortadilatatie of klepperforatie (type 1); prolaps (type 2) of retractie van klepslippen (type 3). De ernst van de aortaklepinsufficiëntie wordt bepaald met de diameter van de vena contracta, de diastolische flowrichting in de aorta descendens, pressure-halftime (p½t ) van de aortaklepinsufficiëntie-jet en kwantitatieve maten zoals effectief regurgitatieoppervlak, regurgitatievolume en regurgitatiefractie. Van deze metingen worden de achtergrond, de uitvoering en de valkuilen besproken.
