Abstract
Ondervoeding is een veelvoorkomend probleem bij kwetsbare ouderen. Ten minste één op de drie geriatrische patiënten kan als ondervoed worden beschouwd.
Ondervoeding staat bij deze groep zelden op zichzelf. Bij ondervoeding zijn er vrijwel altijd ook problemen in het somatische, psychische, functionele en/of sociale domein. De aanpak van ondervoeding is dan ook altijd multifactorieel, gericht op enerzijds de behandeling van het onderliggende probleem, anderzijds de behandeling van de ondervoeding.
Dit hoofdstuk beschrijft de verschillende stappen in de herkenning en behandeling van ondervoeding bij de geriatrische patiënt: werkdefinitie, screening en assessment, behandeling van ondervoeding en de onderliggende oorzaken, rol van de diëtist en transmurale samenwerking.
De inhoud van dit hoofdstuk is tot stand gekomen door middel van consensus van een panel van klinisch geriaters en gebaseerd op de best beschikbare wetenschappelijke bewijsvoering in combinatie met ervaringen en best practices.
