Inleiding
Ongeveer 10% van alle patiënten met een AHA heeft slechts één functionerende ventrikel. De andere ventrikel is altijd aanwezig, maar is rudimentair – soms zelfs in het geheel niet macroscopisch zichtbaar – of zodanig onderontwikkeld dat deze niet effectief kan functioneren. Intra-uterien geeft dit zelden aanleiding tot problemen, daar er in deze fase een parallelle circulatie bestaat: als één ventrikel niet functioneert – het maakt niet uit of dit nu een LV of een RV is – neemt de andere ventrikel dit probleemloos over (figuur 18.1). Pas na de geboorte ontstaat de in serie geschakelde circulatie; LV en RV krijgen in principe elk hun eigen functie. Bij het ontbreken of niet functioneren van één ventrikel komt de circulatie kort na de geboorte in gevaar. Als de ene functionele ventrikel een goede connectie heeft met zowel de art. pulmonalis als de aorta, zal deze ene ventrikel long- en lichaamscirculatie kunnen onderhouden. De verdeling tussen long- en lichaamscirculatie zal afhangen van eventueel aanwezige AS of PS of, bij afwezigheid daarvan, van het verschil in vaatweerstand tussen de twee circuits. Als de ene ventrikel slechts verbinding heeft met één arterie – óf de aorta óf de art. pulmonalis – zal de circulatie die niet direct verbonden is met de ventrikel, alleen bloed ontvangen via de ductus Botalli.
