Chronische myeloproliferatieve neoplasma’s

Inleiding

De chronische myeloproliferatieve neoplasma’s (MPN) kunnen worden onderverdeeld in chronische myeloïde leukemie (CML) , polycythaemia vera (PV) , essentiële trombocytose (ET ) en myelofibrose . Ze worden gekenmerkt door klonale proliferatie van één of meer myeloïde voorlopercellen (granulocytair, erytrocytair of megakaryocytair) in het beenmerg. Deze proliferatie is geassocieerd met een relatief normale uitrijping die leidt tot een toename van granulocyten, erytrocyten en/of trombocyten, in tegenstelling tot bijvoorbeeld de acute leukemie waarbij er juist een differentiatiestoornis is die aanleiding geeft tot ophoping van voornamelijk niet-uitgerijpte myeloïde voorlopercellen. De ziektebeelden binnen de MPN zijn nauw verbonden, er is vaak overlap van klinische, laboratorium- en morfologische bevindingen en in het beloop van de ziekte treedt vaak evolutie op van de ene ziekte-entiteit naar de andere. Soms is deze overlap zo groot dat rubricering niet goed mogelijk is; het WHO-classificatiesysteem spreekt dan van ‘MPN unclassifiable’.
