Samenvatting
De rompmotoriek blijkt een belangrijke prognostische factor van functioneel herstel na een cerebrovasculair accident (CVA). De rompmotoriek kan na een beroerte klinisch snel, betrouwbaar en valide gemeten worden. Met de Trunk Impairment Scale (TIS), met subschalen statische en dynamische zitbalans en coƶrdinatie, is te beoordelen of een patiƫnt kan blijven zitten en wordt tevens de selectieve lateroflexie en rotatie van het bovenste en onderste deel van de romp gescoord. De TIS en de subschaal statische zitbalans blijken bij opname in een revalidatiecentrum de belangrijkste voorspeller te zijn van de score op de Barthel-index zes maanden na een CVA, belangrijker dan de score op de Barthel-index bij opname.
Onderzoek met de Trunk Impairment Scale laat zien dat de rompmotoriek na een CVA een herstelpatroon kent dat vergelijkbaar is met dat van de arm- en beenmotoriek en dat van de functionele vaardigheden. Er blijken geen significante verschillen te bestaan tussen het niveau van motorisch en functioneel herstel na een beroerte. Tussen drie en zes maanden na een CVA blijkt er verder geen significante vooruitgang van motorisch en functioneel herstel meer plaats te vinden, maar zelfs een stagnatie en mogelijk zelfs een achteruitgang. Een gerandomiseerd en gecontroleerd pilotonderzoek ten slotte toont aan dat additionele therapie gericht op het verbeteren van de rompmotoriek een significant positief effect heeft op lateroflexie van de romp, uitgevoerd vanuit het bovenste en onderste deel van de romp.

Testprotocol Trunk Impairment Scale
Voor het uitvoeren van de test zijn nodig: een behandelbank, een blokkussen (om 10 cm achter de patiƫnt te zetten bij item 3 van het onderdeel statische zitbalans) en een stopwatch.
De uitgangspositie bij de 17 items is dezelfde. De patiƫnt zit op de rand van een behandelbank zonder arm- en rugondersteuning. De bovenbenen maken volledig contact met de behandelbank. De voeten worden op heupbreedte en plat op de grond geplaatst. De kniehoek is 90 graden. De armen rusten op de benen. Indien een verhoogde tonus in de paretische arm aanwezig is, wordt de positie van deze arm als uitgangshouding genoteerd. Hoofd en romp zijn in middenpositie.
Indien een patiƫnt een score 0 behaalt op het eerste testitem, dan wordt de totale score van de TIS 0. Elk testitem mag drie keer afgenomen worden. De hoogste score wordt genoteerd. Oefening van een testitem voorafgaand aan de test is niet toegestaan.
De patiƫnt mag tussen de pogingen door gecorrigeerd worden. De testitems worden verbaal aan de patiƫnt uitgelegd en kunnen, indien nodig, worden voorgedaan door de testafnemer. In tabel 7.1 tot en met 4 is het testformulier Trunk Impairment Scale (Van Peppen et al., 2004) weergegeven.
