Abstract
Een 34-jarige man bezoekt het spreekuur wegens vochtretentie en hypertensie. De vochtretentie gaat niet gepaard met dyspneu en is ontstaan in aansluiting op een herniaoperatie twee jaar tevoren. In die periode is hij ook 25 kg zwaarder geworden (BMI nu 36 kg/m2) en heeft hij hypertensie gekregen; bij ambulante automatische bloeddrukmeting was de bloeddruk overdag 160/87 en ’s nachts 179/87 mmHg. Medicatie (o.a. atenolol en Dytenzide) verdroeg hij slecht.
