Zusammenfassung
Een 69-jarige patiënte met IgM-RF-positieve reumatoïde artritis (RA), bij wie de RA al jaren rustig was zonder tweedelijns antireumatische therapie, presenteerde zich elders op de eerste hulp vanwege sinds kort bestaande progressieve dyspnoe. Bij KNO-onderzoek toen geen bijzonderheden. Zij werd opgenomen onder de werkdiagnose bronchopneumonie, waarbij na twee dagen een stridor ontstond. Er bleek een progressieve zwelling te bestaan van de supraglottische regio, mondbodem en tong. Intubatie volgde met opname op de afdeling Intensive Care. Herhaald KNO-onderzoek liet diffuse zwelling zien van de tong, farynx en hals en tevens tracheo-bronchomalacie.
