Natural orifice translumenal endoscopic surgery (NOTES)

Abastract

De ‘natural orifice translumenal endoscopic surgery’ (NOTES) is een paradigma voor een verdere ontwikkeling van de minimaal invasieve chirurgie. De invoering van de laparoscopische chirurgie heeft een ware omwenteling teweeggebracht in de heelkunde, niet alleen van organen in de abdomen, maar ook in het algemene denken over de chirurgische benadering van afwijkingen in het menselijk lichaam. Incisieloze, pijnloze, littekenloze chirurgie werd de Heilige Graal van de moderne minimale toegangschirur-gie. Sinds een aantal decennia wordt een groot aantal ziekelijke afwijkingen via een minimale toegang (lees kleine incisie) benaderd en behandeld. Daarbij rees al spoedig de vraag of conventionele laparoscopische procedures ook zonder incisie uitgevoerd zouden kunnen worden, gebruikmakend van de flexibele endoscooptechnologie en de buikholte benaderend via natuurlijke orificia zoals mond, anus, vagina of zelfs urethra. In experimenten bleek dat zeer wel mogelijk en in september 2007 publiceerde de op het terrein van de minimaal invasieve chirurgie prominent aanwezige chirurg Marescaux uit Straatsburg als eerste in de Archives of Surgery over ‘Surgery without scars’, aangaande een cholecystectomie via een transvaginale benadering. Het postoperatieve beloop was zonder problemen.
