Abstract
Schematherapie is oorspronkelijk ontwikkeld als een individuele Integratieve Cognitieve Gedragstherapie door Jeffrey Young en zijn collega’s (Young et al., 1990, 1999). Elementen uit cognitieve, gedragstherapeutische en psychodynamische modellen, hechtings- en Gestaltmodellen worden binnen de schematherapie met elkaar gecombineerd. Van groot belang is de therapeutische relatie: in en door de therapeutische relatie kunnen vroege schema’s geı¨dentificeerd en veranderd worden. De resultaten van individuele ambulante schematherapie zijn veelbelovend (Giesen-Bloo et al., 2006; Svartsberg, Stiles & Steltzer, 2004). Het argument dat het in deze studies zou gaan om relatief milde persoonlijkheidsproblematiek gaat niet op. Een groot deel van de patiënten, waaronder ook patiënten met een borderline persoonlijkheidsstoornis waarbij er sprake is van een hoge mate van crisisgevoeligheid, zelfbeschadigend gedrag en gebrek aan basisstructuur, zijn goed te behandelen met een tweewekelijkse individuele ambulante schematherapie. Een deel van deze patiënten stopt echter voortijdig met de behandeling of verbetert niet ondanks langdurige intensieve ambulante behandeling met schematherapie. Er blijft een groep patiënten bestaan die om verschillende redenen niet voldoende kan profiteren van een ambulante schematherapie of andere vorm van ambulante psychotherapie.
