Vermoeidheid bij borstkankerpatië nten tijdens en na adjuvante chemotherapiebehandeling

Samenvatting

Vermoeidheid is een van de meest voorkomende klachten bij vrouwen met borstkanker, met name als ze chemotherapie krijgen. Ondanks de hoge prevalentie van vermoeidheid en de diepgaande effecten op de kwaliteit van leven, is weinig bekend over het verloop en de factoren die van invloed zijn op vermoeidheid bij borstkankerpatiënten die chemotherapie ondergaan. In dit hoofdstuk wordt een prospectief cohortonderzoek naar vermoeidheid beschreven, waarvan de resultaten belangwekkend zijn voor fysiotherapeuten die met deze patiënten in aanraking komen. In dit onderzoek werden 157 borstkankerpatiënten vijfmaal geïnterviewd: drie keer tijdens en twee keer na de chemotherapiebehandeling. De meeste patiënten kreg en zes kuren. Dezelfde patiënten vulden ook een dagboekje in om de vermoeidheid tussen twee kuren te meten. Uit de resultaten van dit onderzoek blijkt dat de prevalentie van vermoeidheid (64-82%) in de onderzoeksperiode hoog is. Direct na de start van de chemotherapiebehandeling is een significante toename van patiïnten die vermoeid zijn te zien. Het verloop van de algemene en de lichamelijke vermoeidheid is grotendeels hetzelfde. Er zijn wel verschillen tussen behandelingen met verschillende stoffen. Het verloop van de dimensies reductie in activiteit, reductie in motivatie en mentale vermoeidheid lijkt tijdens de chemotherapiebehandeling stabiel met een lichte verbetering na de laatste kuur. Het verloop van de vermoeidheid tussen twee kuren heeft een wisselend patroon.
