Hoe staat het met het begrip ‘happy wheezer’ en moet elk astma op zuigelingenleeftijd worden behandeld?

Meer dan de helft van alle kinderen jonger dan vijf jaar hebben last van hoesten, piepen en benauwdheid tijdens en verkoudheid. Slechts bij een minderheid persisteren deze klachten wanneer zij ouder worden. De meeste kinderen hebben last van virus geïnduceerde intermitterende benauwdheid.(1) Ongeveer eenderde van hen zal op de leeftijd van zes a zeven jaar nog regelmatig klachten houden. Het zijn deze kinderen waarbij de diagnose astma dan waarschijnlijk gesteld dient te worden. Bij de anderen spreekt men wel van “peuterastma”, of “happy wheezer”. Veel van deze kinderen worden behandeld met inhalatiecorticosteroïden. De vraag is of dit in alle gevallen wel nodig en nuttig is.
