Samenvatting
Het grote verschil tussen endoscopische en open chirurgie is de indirecte benadering van het doelorgaan. Het zicht gaat via een optisch systeem en monitor, waarbij de driedimensionale werkelijkheid tot een tweedimensionaal beeld wordt gereduceerd. Directe manipulatie van weefsel met de handen is niet mogelijk en geschiedt door middel van lange, dunne instrumenten. Mede als gevolg van het verlies aan diepte-inzicht door het tweedimensionale beeld is deze manipulatie lastig. De chirurg is daarnaast beperkt in zijn bewegingsvrijheid, omdat de instrumenten maar via een paar vaste punten (trocarthuls) zijn ingebracht. Ten derde is er geen terugkoppeling van informatie over weefseleigenschappen en dus geen tastzin. Tot slot is er minder controle over de knijpkracht en trekkracht die de chirurg met het instrument op het weefsel uitoefent. Kennis, training en voorzichtigheid zijn dan ook kernbegrippen bij weefselmanipulatie tijdens endoscopische chirurgie.
