In vergelijking met middelen als opium en cannabis, die al sinds de prehistorie gebruikt worden, is xtc een zeer moderne drug. Hoewel dit middel pas echt bekend werd in de laatste twee decennia van de vorige eeuw als brandstof van de dance en party cultuur, is het eigenlijk al bijna honderd jaar oud. MDMA (3,4-methyleendioxymethamfetamine, de chemische naam voor xtc), werd immers voor het eerst gesynthetiseerd in 1912 door de Duitse farmaceutische firma Merck als een onbelangrijk bijproduct in hun zoektocht naar nieuwe hemostatica (bloedstelpende geneesmiddelen). MDMA is dus niet ontwikkeld als eetlustremmer, in tegenstelling tot wat vaak beweerd wordt. In de jaren vijftig van de vorige eeuw onderzocht de Universiteit van Michigan, gesponsord door het Amerikaanse leger, de toxiciteit van MDMA (en nog zeven andere drugs) in cavia's, ratten, muizen, honden en apen. Er is geen bewijs in het publieke domein dat EA 1475 (de codenaam voor MDMA) aan mensen is toegediend of is getest als waarheidsserum (Freudenman et al., 2006). Op het einde van de jaren zestig van de vorige eeuw werd MDMA herontdekt door Alexander Shulgin , een chemicus die verschillende nieuwe psychoactieve drugs uitvond en testte op zichzelf en een groep vrienden (waaronder 2C-B, zie hoofdstuk 13). Verrast door de warme en communicatieverhogende effecten van het middel, introduceerde hij MDMA bij bevriende psychologen en psychiaters (Shulgin & Shulgin, 1991). Gedurende de jaren 1970 werd MDMA ondergronds als hulpmiddel bij psychotherapie gebruikt, maar langzaamaan raakte het middel ook ‘op de straat’ bekend. Samen met het ontstaan van een nieuwe muziekstijl ‘house’ wordt xtc (de naam die MDMA ondertussen heeft gekregen) steeds bekender gedurende de jaren 1980. In 1985 wordt MDMA in de USA via een spoedprocedure door de DEA tijdelijk in Schedule 1 geplaatst en een jaar later wordt dat definitief. Xtc en de rave subcultuur verspreiden zich naar Goa, Ibiza, Londen en uiteindelijk de rest van Europa (Holland, 2001). Midden jaren 1980 duikt xtc voor het eerst op in Nederland. In verschillende kringen wordt er thuis, in besloten kring vanwege de therapeutische effecten, mee geëxperimenteerd. In Nederland hebben de eerste ondergrondse feesten plaats in 1987 en vanaf 1990 begint de dancescene te professionaliseren. In 1988 wordt xtc ook in Nederland illegaal. Ondanks het verbod wordt xtc een heel geliefd feestmiddel in het laatste decennium van de twintigste eeuw. Van de bezoekers van houseparty's en clubs in de jaren negentig van de vorige eeuw had 22 tot 64 procent diezelfde nacht xtc gebruikt (Van de Wijngaart, 1997; Korf et al., 2003). In de 21e eeuw is het gebruik van xtc gedaald en gestabiliseerd, hoewel het na alcohol, tabak en cannabis, de populairste partydrug blijft.
