Abstract
Persoonlijkheidspathologie heeft per definitie te maken met maladaptieve persoonlijkheidskenmerken die stabiel zijn in de tijd. Gezien de veronderstelde stabiliteit van de gedrags- en intermenselijke kenmerken die bij persoonlijkheidspathologie horen, is het aannemelijk dat de oorsprong vóó r de volwassenheid ligt en dat de problemen reeds in de kindertijd of adolescentie zijn begonnen. Er zijn aanwijzingen dat persoonlijkheidspathologie, zoals antisociale persoonlijkheidsstoornis of borderline persoonlijkheidsstoornis, wordt vooraf gegaan door negatieve gezinsinvloeden in de kindertijd (Moffitt e.a., 1996; Zanarini e.a., 1997). De vraag is of er ook voorlopers zijn van de volwassen persoonlijkheidsstoornissen in de vorm van persoonlijkheidsstoornissen in de kindertijd of in de adolescentie. In de kinder- en jeugdpsychiatrische diagnostiek worden op dit moment categorieën van persoonlijkheidsstoornissen vrijwel niet toegepast. Dit heeft waarschijnlijk te maken met het feit dat veel van de diagnostische criteria minder geschikt zijn om gebruikt te worden voor kinderen en adolescenten. Een tweede reden is dat het kenmerkende verschil tussen persoonlijkheidspathologie en psychopathologie bij volwassenen, namelijk het episodische karakter van psychopathologie versus het stabiele karakter van persoonlijkheidspathologie, minder goed toepasbaar lijkt voor kenmerken van disfunctioneren in de kindertijd en in de adolescentie. De meeste psychopathologie in de kindertijd en in de adolescentie kent geen duidelijk begin en is vaak chronisch van aard. Ook kunnen psychiatrische stoornissen in de kindertijd en in de adolescentie beter opgevat worden als extremen van normale variaties dan als welomschreven ziekte-entiteiten. De vraag kan gesteld worden of persoonlijkheidspathologie en psychopathologie bij kinderen en adolescenten wezenlijk van elkaar verschillen. Het is dan ook relevant om in het kader van het onderwerp persoonlijkheidspathologie bij volwassenen de continuïteit en discontinuïteit van psychopathologie in de kindertijd en in de adolescentie aandacht te geven. Dit is dan ook het onderwerp van dit hoofdstuk.
