Abstract
In 1899, bij de zesde druk van zijn Psychiatrie, introduceerde Emil Kraepelin de term ‘dementia praecox’ als een verzamelwoord voor drie tot dan toe afzonderlijk beschreven aandoeningen. De hebefrenie werd al in 1871 door Hecker beschreven, de katatonie in 1874 door Kahlbaum en de dementia paranoides in 1886 door Sander. Kraepelin's beslissing om die drie bestaande aandoeningen samen te voegen tot één nieuwe ziekte-eenheid (die in 1908 door Bleuler omgedoopt zou worden tot schizofrenie) heeft een diepgaande invloed gehad op de psychiatrie. Kraepelin vond destijds dat deze drie aandoeningen iets gemeenschappelijks hadden, namelijk ‘een merkwaardig verloren gaan van de innerlijke samenhang van de persoonlijkheid met een ernstige beschadiging van het gevoelsleven en de wil’. Vanaf het begin was er dus de veronderstelling dat er een verband was tussen de ‘schizofrenie’ en de persoonlijkheid. De uitdrukking ‘uiteenvallen van de innerlijke samenhang van de persoonlijkheid’ werd hier in overdrachtelijke zin gebruikt; niet duidelijk werd hoe men zich dat uiteenvallen van de persoonlijkheid concreet moest voorstellen. Mogelijk komt de term ‘gespleten persoonlijkheid’, die in het verleden vaak gebruikt werd om schizofrenie mee aan te duiden, daar nog vandaan.
