Verslaving en persoonlijkheidspathologie

Abstract

Sinds de invoering van de DSM-III in 1980 met een aparte as (as II) voor persoonlijkheidsstoornissen is er steeds meer belangstelling gekomen voor het onderzoek naar comorbiditeit van as-II-stoornissen bij patiënten met verslavingsproblemen. De drijvende kracht achter deze belangstelling waren en zijn nog steeds de problemen die zorgprofessionals ervaren in het omgaan met en de behandeling van patiënten met deze vorm van comorbiditeit. Hoewel persoonlijkheidsstoornissen al langer het onderwerp zijn van talloze verslavingsonderzoeken, hebben onderzoekers op dit gebied nog maar heel weinig aandacht besteed aan het tegelijkertijd optreden van verslaving en persoonlijkheidspathologie. Deze stand van zaken is moeilijk te begrijpen als men bedenkt dat de combinatie van middelengebruik en persoonlijkheidsstoornissen een van de meest voorkomende vormen van psychiatrische comorbiditeit is. In de meeste boeken over persoonlijkheidsstoornissen wordt het onderwerp middelengebruik maar zelden aan de orde gesteld. Hiervoor zijn verschillende redenen aan te dragen. Ten eerste is het onderzoek naar persoonlijkheidsstoornissen nog maar betrekkelijk kort geleden, in de jaren tachtig van de vorige eeuw, op gang gekomen. In het verslavingsonderzoek werd daarentegen het verband met persoonlijkheidsstoornissen al langer erkend. Zo brachten de eerste twee uitgaven van de DSM de verslavingsstoornissen onder bij sociopathie. Ten tweede hebben instituten en therapeuten, die gespecialiseerd waren in de behandeling van persoonlijkheidsstoornissen, patiënten met een comorbide verslavingsstoornis stelselmatig van hun programma's uitgesloten. De belangrijkste reden hiervoor was ongetwijfeld dat werd aangenomen dat deze patiënten weinig mogelijkheden hadden om te veranderen, niet geanalyseerd konden worden en een hoog risico liepen om het programma voortijdig te verlaten. Ten slotte zijn de financieringsmogelijkheden voor onderzoek naar persoonlijkheidsstoornissen beperkt geweest, tenminste vergeleken bij de financiering van onderzoek naar as-I-stoornissen. Daarom is het merendeel van de studies die zich richten op as-II-pathologie in feite uitgevoerdmet patiënten die voor behandeling van as-I-stoornissen zijn verwezen.
