Imaging of Ileal Pouch Surgery and Related Complications

Abstract

Restorative proctocolectomy with ileal pouch - anal anastomosis surgery represents the surgical treatment of choice for refractory or steroid-dependent Ulcerative Colitis. However, despite refined surgical technique and patients' satisfaction, the procedures, is associated with significant postoperative and long-term morbidity. Most usually secondary to pelvic sepsis, pouch failure leading to reservoir excision and/or permanent ileostomy occurs in 4-10% of patients. Multidetector CT represents the preferred imaging modality to assess patients with suspected early postoperative problems. Magnetic Resonance Imaging (MRI) complements endoscopy in the assessment of pouch-related inflammatory conditions, allowing confident differentiation of uncomplicated pouchitis from pelvic sepsis. After a concise description of the normal ileal pouch appearance, early and delayed surgical / mechanical, inflammatory, and neoplastic pouch-related disorders are reviewed with their cross-sectional imaging findings. Familiarity with usual and abnormal postoperative appearances is necessary to elucidate complex postsurgical complications.
