Abstract

The management of patients with perianal disease, and in particular those with perianal fistula, depends on the careful evaluation of the clinical history, the findings of the physical examination, and an assessment of intestinal diseases and perianal lesions (Fig. 30.1) [1]. These steps are mandatory to plan proper treatment, which may be medical, surgical, or, as is often the case, a combination of the two. Medical therapy mainly consists of antibiotics, anti-tumor necrosis factor alpha, and immunosuppressants, whilst in surgical treatment fistulotomy, fistulectomy, seton placement, endorectal advancement flaps, fecal diversion, and proctocolectomy are the most common procedures.
