Riassunto

Sono le ore 02:00 del mattino, due agenti stanno eseguendo un normale controllo di polizia stradale. Improvvisamente, il conducente di un furgone inizia a sparare verso uno degli agenti. Protetta dal suo giubbotto antiproiettile, l’agente è colpita solo al braccio destro. Il secondo agente apre subito il fuoco e colpisce l’aggressore al torace e all’addome. Sono inviati due ambulanze e un medico d’urgenza1 che arrivano otto minuti più tardi sul posto della sparatoria. All’arrivo, i soccorritori trovano una donna, agente di polizia di 28 anni, vigile e orientata, con un sanguinamento in atto dall’arteria brachiale. Lamenta una completa perdita di forza e di sensibilità del braccio destro. Non sono evidenti altre ferite. I soccorritori applicano un bendaggio compressivo e il sanguinamento si ferma. La pressione arteriosa è 95/50 mmHg e la frequenza cardiaca 90 bpm. Il medico è impegnato con l’agente di polizia per la ricerca di un accesso venoso e chiede a un soccorritore di valutare l’automobilista ferito che giace vicino al suo furgone. L’aggressore è tachipnoico, obnubilato, con polsi periferici deboli. Su ordine medico, riceve ossigeno in maschera, accesso venoso e comincia il riempimento vascolare. Una volta caricata la poliziotta nell’ambulanza, il medico dedica la sua attenzione all’aggressore, che nel frattempo ha perso conoscenza. L’esame clinico ritrova diversi fori d’ingresso di proiettili, a livello del torace e dell’addome. Il polso centrale è debole e rapido. Il paziente è immediatamente intubato. All’auscultazione del torace, il murmure vescicolare è assente dal lato colpito dai proiettili. Un enfisema sottocutaneo si sviluppa rapidamente dallo stesso lato.
