Abstract
Oncoplastic surgery represents an important evolution, allowing a better aesthetic–functional outcome for patients with breast cancer. The emphasis of this new phase in breast surgery is on immediate reconstruction associated with the transverse integration of oncologic and aesthetic–functional concepts by the surgery team and by the single surgeon. Therefore, preoperative planning is the most important time in this surgery, with two fundamental aims: anticipation of the surgical steps to follow in the operating theater, and reduction of surgical risks. In planning oncoplastic surgery it is essential to anticipate the size and location of future glandular and skin defects, and the relation of the nipple–areola complex to them. One must also plan how to correct possible previous asymmetries too, and to combine this with the oncologic approach. In this chapter, we discuss decisions regarding oncoplastic surgery and surgical planning in class I and class II surgical techniques in order to achieve the best oncologic and aesthetic–functional outcomes, and how to reduce errors.
